COUNSELORS (OUTPATIENT & AFTERCARE (Position’s available immediately)

LOCATED AT 555 STOCKTON STREET & SULZBACHER CENTER FOR THE HOMELESS

Criminal Background, TB Test & Pre-Employment Drug Screening required

 

Provide both direct and indirect services to clients in a multidisciplinary treatment team. Services provided include screenings, assessments, orientation, case management, individual  and group therapy as well as treatment planning. Demonstrate competencies in client population served. . Assist Director of OP Services as needed.

ESSENTIAL FUNCTIONS

  1. Ensure quality internal and external customer care and service.
  2. Provide screening and assessments and make recommendations.
  3. Provide orientation, treatment, individual and group therapy, case management, and discharge planning services.
  4. Ensure completeness and timeliness of clinical documents and administrative forms to include credentialing once CAP is obtained.
  5. Demonstrate flexibility as a team member and promote a positive working environment.
  6. Make an attempt to contact family/social contacts of each client to gather corroborating information, if applicable.
  7. Complete GAIN  and/or BPSA
  8. Learn how to utilize agency’s electronic health record: AVATAR
  9. Attend bi-weekly OP team meetings/supervision.
  10. Perform peer review assessments monthly
  11. Attend Medical Staffing with Medical Director as needed.
  12. Perform special projects and other Agency related assignments as requested.

PERFORMANCE STANDARDS

  1. Must pass required background investigation.
  2. Must complete all required training.
  3. Must abide by Agency's Code of Ethics, Agency Standards, Policy and Procedures Manuals.
  4. Must practice universal precautions as part of regular job duties whenever applicable.
  5. Must possess competencies required to effectively understand and deal with population served.
  6. Must complete 20 hours job related training annually.
  7. No more than 5% error rate in AVATAR documentation striving for 0% error rate. All errors, once identified, must be completed within 3 working days.
  8. Must be able to perform assigned work independently with minimal supervision.
  9. Timely and regularly complete all required documentation, reports, and other Agency related paperwork.
  10. Ability to function as an effective team member.
  11. Maintain the work area in a clean and orderly condition.
  12. Ensure safe operating conditions within the area of responsibility.
  13. Must not engage in behaviors that erode the cohesiveness of the program staff.

     

  • Master’s degree in related field and experience in the treatment of behavioral health disorders
  • CAP Preferred
  • Ability to engage in critical thinking.                
  • Ability to formulate professional clinical reports.
  • Must possess time management skills.
  • Ability to help with problems areas without being overly critical.
  • Ability to recognize and respond to emergencies and life threatening situations in an appropriate manner. 
  • Ability to work in fast-paced environment.
  • Strong customer service and organizational skills.
  • Ability to relate to, adults, families, referral sources and internal Gateway Team Members and must possess requisite competencies to effectively understand and provide appropriate services to the client population served.
  • Ability to relate and present to groups.
