Patient Care Manager

Wekiva Springs Center is a 120-bed acute behavioral health hospital providing quality care in a gender-specific program for men and women. We provide treatment for mental health, addictions and dual diagnosis concerns in a warm and caring atmosphere near I-95 and J Turner Butler in Southside area. 

The Patient Care Manager is a member of the multidisciplinary treatment team. Major job functions include: conducting psychosocial and CD assessments; including gathering info from family and other providers. Identifying discharge needs, barriers to discharge, and complete discharge planning. Assisting with placement and other discharge planning needs; including scheduling appointments for aftercare. Completing documentation per policy and procedure, including treatment plans and treatment plan updates. assisting in developing and maintaining community resource information; performing marketing functions as assigned; assisting in the maintenance of the program environment in compliance with policies of safety and security; ensuring good clinical judgment to ensure optimum patient care; participating in continuing education activities.

 

Duties include:

  • Completes and documents assessments per hospital policy including psychosocial assessments/CD assessments and other assessments as needed
  • Completes progress notes per hospital policy 
  • Assures all case coordination is completed and documented in the chart
  • Assists with completing releases/consents as needed from patient and/or family
  • Formulates discharge plan with the treatment team and family as an integral part of treatment
  • Charting is clear, concise, and reflects weekly treatment plan goals/objective on a timely basis
  • Schedules aftercare appointments within required timeframes
  • Completes Master Treatment Plans/ Treatment plan updates per hospital policy
  • Attends Multidisciplinary treatment team meetings 
  • Provides group therapy to patients when needed
  • Provides feedback to therapist on problems listed on treatment plan
  • Ensures that therapies conducted are consistent with the goals and philosophies of the program and supportive of each patient’s individual treatment plan
  • Documents referrals that are made for services not provided by the facility as needed
  • Assures that changes in diagnosis, legal status, medical issues, identified problems and discharge plans are discussed with the therapist and documented in the medical record
  • Maintains and exhibits knowledge related to the treatment of adults in an acute setting
  • Exhibits knowledge and skill related to group work with adults
  • Demonstrates an understanding of specific issues related to patients from age 18 through the older adult population
  • Demonstrates knowledge of external service which can be accessed for adults upon discharge
  • Provides dual diagnosis care, as deemed necessary
  • Identifies alterations in sensory perceptual images, recognizing the differences between illusions, delusions and hallucinations
  • Provides assistance to nursing/admissions staff when they are assessing an individual
  • Provides assistance with mentoring/training new staff and interns and assists coworkers willingly as needed.
  • Performs duties as assigned by Program and Department Director

 

Demonstrates knowledge of DSM-5 diagnostic criteria and the treatment of:

  • Somatoform Disorders
  • Mood Disorders
  • Dissociative Disorders
  • Gender & Sexual Disorders
  • Adjustment Disorders
  • Personality Disorder
  • Chemical Dependency
  • Anxiety Disorders

 

EXPERIENCE/EDUCATION:

Must be licensed in the State of Florida or licensed prepared as an LCSW, LMHC, or LMFT. Master's Degree from an accredited school in the Behavioral Sciences; three (3) years post Master's working experience including of group therapy, case coordination activities, substance abuse, mental health and specific treatment related to adults in acute care and/or intensive outpatient settings. Must meet State Law regulations for credentialing clinicians. Unlicensed therapists must be Master’s prepared, and actively working toward licensure. Weekly they should receive documented supervision and their clinical entries are counter-signed.
