BIG BEND HOMELESS COALITION
Housing Case Manager
We are looking for experienced case managers for a range of programs - Rapid Rehousing, Homeless Prevention, and Permanent Supportive Housing. A successful candidate will not only fulfill qualifications and demonstrate required skills, but also bring a trauma-informed awareness to our strength-based organization. An exceptional ability to use de-escalation and conflict resolution skills is required along with a value for diversity, a willingness to self-assess, and the capacity to adapt to diversity and context.
General Description: To provide case management services to Veterans, families, and individuals with the primary objective of assisting participants in achieving housing stability.
- Essential Functions:
- Provide crisis intervention and case management related to housing stability
- Assess needs
- Coordinate and assist with housing placements
- Provide housing-related information, education, and referrals on issues related to housing stability and basic needs
- Provide in-home visits for initial intake and follow-ups.
- Develop individualized Housing Stability Plans
- Provide advocacy on behalf of clients and families
- Develop and maintain cooperative working relationships with the community partners, such as the VA, CoC, THA, and others
- Coordinate supportive services between participant and community service providers
- Working Conditions: The position may require travel to rural communities’ of the Big Bend, including but not limited to Gadsden, Wakulla, Liberty, and Franklin Counties, and occasionally requires working outside of regular 9-5 work hours.
- Qualifications and Skills:
- Master's in Social Work
- Excellent interpersonal skills and demonstrated ability to work with marginalized or underserved client populations,’ e.g. experiencing homelessness
- Knowledge of community resources and ability to foster working relationships with colleagues, clients, and community agencies
- High level of professionalism and ability to work independently
- Strong organizational, time management and communication skills
- Computer proficient
- Excellent interpersonal skills, including the ability to manage crisis situations
