As a Federally Qualified Health Center (FQHC), Covered Bridge Healthcare of St. Joseph County has been federally grant funded under section 330 of the Public Health Service Act (PHS) – to provide patients access to the highest levels of care. Our services include quality, affordable care to the community with expansion of care to the underserved in a collaborative manner. We focus not only on primary care, but the whole person, Behavioral Health, Substance Use, Social Determinants of Health, and much more.
Covered Bridge Healthcare of St. Joseph County qualifies for reimbursement from both Medicare and Medicaid and offers a sliding fee scale to defer costs from our patients. We serve all ages for both medical and behavioral health.
Transportation will be necessary to get to and from CBH. The agency is accessible by bus.
· Video/Audio taping is permitted with patient/staff permission.
· Clinic hours: M,T,W,F 8am-5pm Thurs. 9am-6pm
· The clinic is not open on weekends or evening, but hours will be available during this time through things such as trainings.
