Sanitas Medical Center is a Primary Care Facility with over 70 clinics in the state of Florida. Sanitas has a behavioral health program called "Bewell," which comprises of both counseling and SUD-related case management. With the SUD Case Management Program, Behavioral Health Case Managers
(BHCMS), work to provide tailored behavioral health support for patients with a substance use disorder. Patients over 12 will be screened and assessed by the PCP for a SUD. Danna will then work with the patient to provide linkage to a higher level of care depending on the results of the
secondary biopsychosocial assessment completed by the BHCM. The BHCM will follow the patient and provide the necessary resources for the patient's improved outcomes. This includes linkage to psychiatry for co-morbidity and or dually diagnosed patients, counseling, detox, and residential.
PHP, IOP, OP, or sober living facilities. Danna will also work to ensure she can have clinical discussions weekly via a multidisciplinary approach with medical professionals. Notwithstanding, Danna will have exposure with supporting patients' recovery goals, including linkage to housing and support with linking patients with higher acuity of mental health with SSI, SSA, SNAP, and other community programs/resources. Mainly, Danna will work to ensure SUD patients are on the road to recovery through her proficient and culturally competent support.
