The Population Health Services Organization includes a variety of roles that function to support primary care patients in the state of Vermont. This field placement will take place specifically at this clinician’s assigned primary care practice, University of Vermont Children’s Hospital Pediatric Primary Care.  Our clinic has locations in Burlington and Williston Vermont, and serves children from birth to age 22, or until their transition to adult primary care. UVM Pediatrics is a teaching practice for Larner School of Medicine, and employs attending physicians, nurse practitioners, RN care coordinators and integrated mental health team of psychologists and social workers. UPeds hosts multiple professionals-in-training per year, including resident physicians, medical students, nursing students, psychology, and social work interns. In addition to primary care, our clinic houses the New American Clinic for Vermont, Developmental-Behavioral pediatrics, Transgender Youth Program, and eating disorder specialty care. The PHSO social worker provides low-barrier access to care coordination in support of patients’ physical and mental health needs and educational goals. Services are provided at no cost to families, and the PHSO social worker/ intern do not bill insurance for care.
Intern Job Description:
-    Field instructor will provide orientation to the larger agency, PHSO, and clinic, and will facilitate intern’s completion of agency orientation requirements
-    All equipment needed for position will be provided, including an agency laptop, cell phone, and access to email, printing, scanning, and office supplies
-    Training in clinical tasks, including mental health and social determinants of health screening, with opportunities to shadow field instructor and other agency professionals as needed
-    Assigned “caseload” of 4-6 patients at a time; PHSO social worker provides “episodic care,” so may engage with a family to complete goals, then “graduate”
-    Intern will participate in weekly interdisciplinary meetings, weekly supervision with field instructor, and any appropriate community or school team meetings for patients
-    Depending on learning goals, intern may participate in other agency committees and trainings, or as an agency representative to a community committee

Special Placement Information:

Agency is accepting MSW Part-time, The Burlington clinic is located on the busline, though intern would need transportation to access
the Williston office. Agency hours are Monday- Friday, 8-4:30, and the intern can complete hours outside of these times as approved by field instructor. The placement can be flexible through remote work as needed. Historically, social workers and interns have attended in-person school and community meetings and home visits, requiring transportation. Due to COVID-19 restrictions on in-person meetings, social workers are only attending virtual meetings at this time.  UVM Medical Center requires COVID-19 vaccination for all employees.
