Information

Program serves all youth ages 16-26 in NE Florida who have childhood onset conditions such as spina bifida, cerebral palsy, sickle cell anemia, down syndrome, autism,etc. The goal of the program is to assure that all youth with special health care needs who are no longer appropriate for pediatric-based care are not lost when they begin the transition to the adult health care system. Some services include: primary care, comprehensive intake, multi-disciplinary evaluation, individualized health care planning, coordination with educational/vocational transition services and assistance securing health insurance coverage and other funding resources, referrals, linkages to community providers.

Special Information: 

BSW and MSW students, clinical.  Health science center is accessible by bus.  Students who use personal vehicles for community or home visits will be reimbursed at current state rates.  Liability insurance is needed. Audio/video taping may be permitted with client/parental consent.  Office hours are M-F 7am-4:30pm
