St. Vincent’s Medical Center is a 512-bed acute care facility in an urban area serving the north Florida and south Georgia regions. Social work services are provided to inpatients and outpatients of St. Vincent’s Medical Center. Outpatients include patients in the Emergency Department, Family Medicine Center, Pre-Admission Testing, Cardiopulmonary Rehab, outpatient surgery, all other Medical Center departments and the community when calls are received. Social work services are provided to all age individuals with a wide variety of conditions and diagnoses. Patients are identified for services through a high-risk screening process and referrals from other health care professionals. Department policies define high and medium risk patients and identify the referral process from the care manager to the social worker. Social workers provide a variety of services, including but not limited to psychosocial and discharge planning services that enable adaptation to the illness and facilitate post hospital planning. Social Work staff collaborates with the interdisciplinary health care team, the patient, and family members in assessing needs and planning future care to ensure that gains achieved from hospital care are maintained or enhanced for the continuing health and welfare of the patients following discharge. The most common services provided by Social Work staff include: discharge planning, community referrals, information, assessment, problem identification, financial assistance, transportation, counseling, crisis intervention.
Special Information:
MSW FULL-TIME PLACEMENT. Student will need transportation to get to and from the agency, but not for purposes of the placement. Students need liability insurance. Video and audiotaping of work with patients is not permitted; it would be permitted, if necessary, with supervision. Hours are Monday-Friday, anytime between 7:30 am and 5:30 pm. No weekend/evening hours are available.
