We serve an array of different populations from geriatrics, pediatrics, and more. FMC strives to provide excellent care to a range of diverse populations. FMC works with counties throughout Northern Arizona including Coconino, Navajo, and Apache counties. The role of social work is to assess, plan, implement, monitor, and measures effectiveness of interventions to meet patients’ needs. A social worker at FMC proactively screens and assesses the transitional needs of the patient. A social worker ensures timely and appropriate discharge for patients. The social worker collaborates with the team to identify, assess, plan, implement, coordinate, monitor, and evaluate options and services to meet the complex needs of patients. A clinical social worker provides supportive counseling, crisis intervention, case management, and community resource referrals. A social worker’s role is to provide emotional and social support during active treatment and recovery period. A social worker may assist in making sure patients are meeting their emotional and psychological needs, performing assessments, crisis interventions, and addressing social and financial concerns, coordinating a plan with patient/family, and providing information and referrals as needed. Case management duties can include referrals to home care, hospice, and other community agencies.

Students will need to provide their own transportation to and from the agency. Travel is not reimbursed. Students will not be paid a stipend. Work hours are Monday-Friday, day time hours.
