West Georgia Care is an agency who provides health & social services to patients in their homes.  Most (but not all) of their patients are over the age of 50.  many have been discharged from the hospital following an acute illness or a planned or elective surgery(ie total knee or hip replacement).  The primary function of the agency is to provide medical care to patients in their homes allowing them to recover at home with family and other support nearby.  The Social Worker conducts biopsychosocial assessments for each referred patient an use this as a tool to identify the strengths and needs of each patient and their support system.  Social Workers provide supportive counseling to help with adjustment to a new diagnosis, condition, accident/injury, or escalation and advancement of an existing disease.  The Social Worker is also responsible for providing information and referral to community resources such as financial assistance or nursing home placement.

MSW 1 year and 2nd Year/Advanced Standing, clinical only. Full time and part time positions available. There is no stipend available. Students will not need transportation.  They will ride with their MSW supervisor to conduct home visits.  Students should provide their own liability insurance. Video & audio taping of clients is not allowed.  Hours for the agency are Mon-Fri 8:00am - 5:00pm. Working four 10 hour days is possible.
