DeSoto Memorial Hospital is a not-for-profit rural hospital Our goal is to provide community members 'With the highest level of healthcare services. The primary population is adults 18+. Children are seen in the ER and transferred to a pediatric receiving hospital should their needs require it. The social worker's role is primarily case management while also addressing social issues that arise (homeless, substance abuse, DCF, etc.). The social worker provides community resources, works with the interdisciplinary team, and works within the community. The case management role includes ensuring the conditions of participation with CMS are met, SDH screening, discharge planning, post-acute care referrals, DME referrals, and transferring patients as needed.

The Agency accepts BSW full-time, MSW 1st year students, MSW 2nd year and Advanced Standing Clinical Students. The hours available are Monday through Friday 7:30 am to 4:00 pm.
