Liberty Resources is an integrated non-profit agency, focused on providing whole-hearted health Within one system of care. Liberty Resources brings together medical care, behavioral health services, and social services under one agency to provide an Integrated, individualized, and whole­ health plan to all individuals served. In a general sense, Liberty Resources provides commu1ity and clinic-based behavioral and substance use healthcare, primary care services, disability services, early childhood intervention, and child and family services within the community across New, York state. In regard to this specific internship, this student will be interning with our adult Health Home Care Management program within Liberty Resources' community-based services division. "This program works with individuals 18 years and older with chronic qualifying conditions ranging from physical health conditions, serious and persist mental health diagnoses and substance use disorders. Individuals present with a diversity of needs Including legal. housing, educational Vocational. social-emotional needs, and family relationship needs. The care manager is critical in assessing for these needs, developing a strengths-based and person-centered plan of care, and providing active interventions to assist the client with progress towards their goal These interventions including connection and referral to necessary supports and providers within the community, addressing gaps in medical and behavioral healthcare to decrease emergency room and hospital utilization, and ongoing collaboration with all care team members to functioning in variety of settings.

 

Special Placement Information:

Agency is accepting BSW full-time, MSW 1st year students, MSW 2nd year and Advanced Standing Students, Part-time BSW, and Part-time MSW Part-time.  In regard to an internship within the care management program at Liberty Resources, interns are required to rave their own transportation as transportation is a requirement for daily responsibilities. Intern will be required to travel to meet with their clients at a variety of locations across their assigned county. Travel reimbursement is not provided to interns, nor are there stipends available. Interns are rot provided with any liability insurance through Liberty Resources, and if such insurance is requested, this would need to be obtained either through their school program or through another personal avenue. Care Management hours are Monday through Friday, typically between the hours of 7:00am to 5:30pm. There are no weekend hours available. Depending on the location where the intern is assigned, would determine accessibility by bus. These details may be different pending an intern's participation in a different program within Liberty Resources.  Care Management is unable to accommodate clinic areas of internship needs as this program does not provide clinical Intervention with clients. However, there are their programs/departments within Liberty Resources that could provide clinical hours.
