Sea Mar patients of all ages qualify for the Care management program based on a criterion of Predictive Risk Scoring, Emergency Department utilization, and chronic conditions such as Asthma, Diabetes. Heart Disease, Cancer, Cerebrovascular Disease. Coronary Artery Disease, Dementia, or Alzheimer's Disease, HIV/ AIDS, Renal Failure, Chronic Respiratory Conditions, Neurological Diseases as well as mental health issues, social barriers and other areas of client risk.   Care Management social workers   provide   six   main   core   services:   Comprehensive   Care   Management   Care Coordination, Health Promotion, Comprehensive Transitional Care from inpatient to other settings, Individual and Family Support Referral to Community and Social Support Services.    
 

Special placement information:

Accepting MSW 1st-year students, full-time only. In this placement the required technology needed to complete expected tasks. such as laptop, work phone. and work space, will be provided by the agency. Any travel needed for duties. such as client meetings, will be reimbursed by the agency. Given that the student  will  be completing  the internship as a part of the student's normal work duties, the student will earn regular work compensation. As a requirement of this position, the student is required to undergo an annual background check, have a valid Washington State driver's license. and valid motor vehicle insurance. Agency hours are Monday through Friday 8:00 am to 5:00 pm.
