Transitions Supportive Care is a community based palliative care program that provides symptom focused care to patients living with serious illness, irrespective of their diagnosis, prognosis, or treatment plan. The mission of Transitions Supportive Care is to improve the patient and caregiver’s quality of life by providing an extra layer of treatment and support. We are made up of an interprofessional team comprised of nurse practitioners, a licensed clinical social worker, and community resource navigators. We serve patients in eight counties: Leon, Jefferson, Wakulla, Madison, Franklin, Taylor, Liberty, and Gadsden.
Special Placement Information
BSW and MSW 1st year students only. Accepting full-time interns only. Interns will need to have transportation to travel to patients’ homes. Mileage is reimbursed at .58/mile. No stipend is offered at this time. Office hours are Monday through Friday, 8 AM- 5 PM. Prior to starting their internships, interns will need to complete a level II background check and a TB test, both of which Transitions Supportive Care will pay for.
