Union Hospital Pop Health Dept. primarily serves Traditional Medicare patients.  Our dept. (RNs and SWs) coordinates community resources to best meet patients’ needs and assist patients to achieve their health care goals.  We work closely with primary care Drs and our community-preferred providers to also reduce Medicare spending, and use patients’ benefits appropriately.

MSW 1st year students.  The student can complete home visits independently, will need to use personal care.  The student will need to complete hours during the workweek, no evening or weekend hours available.
