-providing end of life care to patients of all demographics in a location being wherever someone calls home. We take care of those who have a life expectancy of 6 months or less should a disease process run its expected course. We work as part of an interdisciplinary team, who treats the whole person. The role of the social work includes but is not limited to goals of care planning, end of life discussions, durable power of attorney and living will guidance, helping to figure out finances, care options, pre-bereavement support as well as support to families/caregivers after a patient passes.

Special Placement Information:

All employees and students will use their own cars for travel. For the agency, we reimburse gas for employees at $0.47 a mile. We also reimburse for tolls incurred during work travel. The office is open Monday-Friday 8:00am-5:00pm but because of the nature of what we do, we do have nurses working 24/7 and do offer on-call social work coverage.
