Boys Town North Florida offers a variety of services based on the research-proven Boys Town Model. Our Family Home Program provides a family for children ages 12 to 18. Six to eight boys or girls live in single-family homes with a married couple called Family-Teachers®. In these homes, youth learn social and independent living skills that help them find success in school and in their community.
Youth and their family are involved in services to support permanency. Our Foster Family Services provide trained and supported resource families that open their homes to children who need a place to safely live. Youth of all ages are served. In-Home Family Services partners with parents and their children in learning productive and supportive ways to prevent difficult situations and learn to manage difficulties in daily living in their own homes. This intensive service aims to keep families together. Community-based services include a wide range of resources to learn and empower parents and children to get the assistance they may need. Care Coordination Services provides a lifeline. The Agency will work with the University in providing learning experiences for students that meet applicable Agency standards, that meet the stated objectives of the University's Educational Experiences, and that effectively facilitate the student's learning process.

consideration of the educational learning experience with the Agency, being made available to me under the MOU, to which this Confidentiality Statement is being attached as an Exhibit, hereby recognize that, while with the Agency, medical records, client care information. personnel information, reports to regulatory agencies, and conversations between or among any healthcare professionals are considered privileged and should be treated with utmost confidentiality. I agree, under penalty of law, not to disclose to any person or persons, except authorized clinical staff and associated personnel of Agency, the above-listed information and further agree not to reveal to any third party any confidential information of Agency, except as required by law or as authorized by Agency.
Special Placement Information:
The Agency is accepting BSW Full-time, MSW 1st year students, MSW 2nd year and Advanced Standing Students. The students who will be receiving experience under this Agreement are not employees of the Agency and are not eligible for compensation or benefits of any kind. Agency hours are from 8:30 p.m. to 5:30 p.m. with additional casework after hours based on the client's needs. Students must sign a statement of responsibility which includes assuming all risks unless arise solely from willful misconduct of the Agency; abiding by the Agency Code of Conduct, policies, procedures, and regulations; and providing background screenings, and health records including proof of required immunizations. See the attachment regarding background screenings. The University will maintain medical malpractice insurance for the student intern per Agency and University requirements.
