Concert Health is a virtual, Behavioral Health Medical Group partnering with Primary Care Practices nationally, providing Collaborative Care and Suicide Safer Care. Collaborative Care (CoCM) is an evidence-based model developed at the University of Washington to address symptoms of the most commonly diagnosed behavioral health concerns, such as depression, anxiety, and substance misuse. CoCM, with over 80 randomized trials in both the United States and Europe, can effectively address, symptoms of the aforementioned concerns in ages 12 and up. The care provided is tracked in a registry, is measurement-based, and offers treatment to target. The Collaborative Care Clinicians work with the Primary Care Provider and the Psychiatric Consultant to provide evidence-based medication in addition to the types of treatment offered in CoCM (brief check-ins, medication adherence, and talk treatment. Collaborative Care billing is monthly through the patient’s primary care provider allowing for greater access to behavioral health care. Social workers at Concert are in the role of Collaborative Care Clinicians providing direct patient care (www.aims.uw.edu). Additionally, a large portion of Concert Health’s leadership team are licensed social workers, as is the Co-Founder and Chief Clinical Officer.
Special Placement Information:
The Agency is accepting MSW 2nd year and Advanced Standing Clinical Students, (Social Leadership), (Clinical) Part-time MSW. Concert Health is a remote company and has been since it’s inception in 2016. Students are expected to have their ‘own’ computer or if they share a home computer, a password can be placed on the computer to protect HIPPA information. Students are expected to have a designated space where they can work privately without violating HIPPA. Finally, good-enough internet access is required in order to effectively work from home.
All students complete onboarding training which is synchronous and asynchronous during the first month of placement. All students are required to attend Student Seminar on Mondays from 12:00 pm – 2:30 pm. We cannot accommodate students in the evenings (later than 5 pm) or on weekends.
