Crossroads Health and Wellness Center is a community behavioral healthcare agency providing Primary Care, Mental Health, Substance Use Treatment and Case Management services. Crossroads Health and Wellness Clinic (CRHWC) provides these services utilizing an Integrated Services model. Primary Care services are provided to patients from birth through geriatrics. Clinical services and case management serve clients from age 5yrs through adulthood. In addition, CRWHC provides 24/7 crisis response services in Duval County and operates these services in conjunction with the 988 Emergency Response Network.
 

Special Placement Information:

Agency is accepting MSW 1st year students, MSW 2nd year and Advanced Standing Clinical Students, (Clinical) Part-time MSW. 
•    CRHWC operates Monday-Friday 8:30am to 5:00pm and Saturday from 9:00am to Noon by appointment. While the agency is accessible by public transportation, it is advisable that students have an independent mode of transportation. Mileage is reimbursed when personal transportation is utilized for business purposes.
•    Video and/or Audio taping is permitted but strictly managed Crossroads clinical leadership.
•    Individual liability insurance is not required for students. However, students are free to obtain liability insurance at their own expense.
