Information

ElderHelp is a nonprofit that has been keeping San Diego seniors in their homes for years. We provide case management, housing, information, referral, and volunteer services geared towards improving social determinants of health so that adults, age and older, can continue to live in their homes and communities. Our program staff work in social work capacities to assess clients and their homes to provide appropriate services and supports to our clients. Though the focus of our services is on the older adult population, our Home Share program serves adults of all ages, matching them to live with seniors. Some of our clientele is homeless or at risk of homelessness, many are at risk of falls and have multiple chronic conditions. Some of our clients are veterans. Our client base touches many different, high-risk populations and our services work to mitigate the challenges these populations face, especially as they age.
