This agency provides support coordination services to the developmentally delayed ranging in age from four through adults. The worker is responsible for the integration and planning for all resources, services and supports that meet the desires and needs of the population, as well as monitoring of support and service delivery.

BSW or MSW, full or part-time placement. Student car required. Daytime hours, weekend and evening can be available if necessary.
