Social Workers see hospice patients primarily providing end of life, caregiver support, financial education, advanced directives, and caregiver resources.
Intern job description:
Administer social services to patients requiring intermittent professional services and teaches the patient, family, and other members of the health care team. These services are performed in accordance with the physician's orders and the established plan of care.
Perform patient visits and ensure visit meets needs of the patient.
Contact physician regarding patient changes; receive and write verbal orders.
Notify physician and designated branch staff of significant changes in patient status; complete documentation.
Report need for add-on services to branch director and clinical supervisor.
Coordinate patient care.
Use appropriate community resources; either refer patient or act as an intermediary on behalf of the patient.
Assist patient and their families with difficulties.
Cooperate and assist other health care providers.
Ensure missed patient visits are rescheduled in the same Medicare week in order to adhere to the patient's plan of care.
Attend and participate in continuing education to improve care.
Notify supervisor of documentation that needs review.
Special placement information:
Accepting BSW students. Full-time positions only.
Travel reimbursement available.
Liability insurance not required.
Video-/Audio-taping permitted with patient/family consent.
Agency hours: 8:00AM-5:00PM, Monday-Friday.
