Encompass Rehabilitation Hospital provides inpatient rehabilitation to patients primarily over the age of 18.
Encompass primarily serves the elderly community and is essential in providing post-acute care in transitioning patients back to their home environment.
Social Workers are part of the case management team at Encompass and focus on discharge planning which includes arranging services for patients and families including home health care, durable medical equipment and follow up appointments in order for patients to successfully discharge back to the community. The Social Worker is a part of the patient’s interdisciplinary team and provides communication between the patients team as well as communication from the team to the patient and family.

Weekend and evening hours not available.
Agency hours: 8:00AM-5:00PM, Monday-Friday.

All students are required to provide the necessary documents for clearance and must attend 1 hour hospital orientation with our Employee Health educator prior to being allowed to start their rotation. Requirements are reviewed by the hospital’s Human Resources Department and approval is needed before the student can begin their rotation.
