First Choice Hospice provides in-home hospice services to patients diagnosed with a terminal illness who have a life expectancy of six months or less by using a multidisciplinary team consisting of physicians, RNs, LPNs, HHA, Social Workers, chaplains, homemakers, volunteers, and the agency medical director. 
The Social Worker's role includes but is not limited to psychosocial assessment of patients and families, documentation of patient care, coordinating access of community resources, communicating patient needs to other members of the team, providing emotional support to patients/caregivers, providing bereavement services to caregivers after patient's death, working with team to plan and present memorial service annually, and assisting with financial needs and resources.

Accepting part-time MSW students.
Transportation required. Travel reimbursement available. Agency not accessible by public transit.
Liability insurance required.
Stipend not available.
Agency hours: 8:00AM-4:30PM, Monday-Friday.
Video-/Audio-taping permitted with consent.
