Health Connect America offers an Innovative approach to treatment and addresses the multiple needs of children, individuals, and families. Our goal is to support the preservation of children, individuals, and families at risk and develop long term stability and improved lives. Health Connect provides intensive in-home services at a minimum of three days weekly, as well as Outpatient services, Intensive Outpatient Services, Medication, Management, and Health Link Services to its risk population.

Accepting BSW, MSW 1st-year, and MSW 2nd-year/Advanced Standing (SL + clinical) students. Full- and part-time positions available.
Students are expected to drive their own vehicle, maintain a good driving record, and maintain vehicle insurance and registration, as they will be using their vehicle to make home visits. Agency accessible by public transit.
Agency hours: 8:00AM-5:00PM. Weekend and evening hours available as the student will make their own schedule when conducting home visits.
There is also an IOP group that meets at the office on Monday, Tuesday, and Thursdays from 4:30PM-7: 30PM.
