LAHR and Columbine Health Systems aims to provide healthcare and appropriate living environments for older adults in Northern Colorado. Patients and residents served in the Columbine Health System range in age from 55 to 100+ years old, experiencing various need for extra support from healthcare providers. The role of the social worker in the skilled Nursing setting is to provide emotional and psychosocial support to residents, and to act as a patient advocate to ensure needs are being met. The social worker completes a comprehensive resident assessment upon each new admission to determine individual care plans including Addressing psychosocial needs including cognition, communication, mood, and expressions of need. Arrange mental health services-psychologist, chaplain, and supportive services surrounding mental health diagnoses/psychoactive medications. Coordinate discharge for short-term rehab patients, including set up of in-home care, medical equipment, and community resources. Manage concern/resolution process and involve community ombudsman, Adult protective services, etc. Track and coordinate ancillary services for residents: dental vision, hearing, podiatry. Monitor advanced directives DNR, power of Attorney. Living will. Most importantly social workers work closely with community partners in order to help patients & their families navigate resources outsides of the facility.
