Chapters Health, Inc. includes three hospice affiliates in the Tampa area.
Our hospices provide end of life care to those affected by life-limiting illnesses utilizing a team approach to care.
Social Workers are an integral part of the team and provide psychosocial assessment and support, bereavement counseling, discharge planning, and a variety of services to assist patients and families navigate end of life issues. 
Age range of patients and bereaved clients is 0+.
Hospice services are provided in the patient’s home, nursing home, assisted living facilities and in our 7 inpatient care units throughout our seven county service area. We also provide a large range of bereavement services, which includes Children's Grief Centers and a weekend camp experience.

Transportation required.
Stipend not available.
Liability insurance required.
Background screening, TB test, flu shot during flu statement, and a statement from their physician that they are free from communicable disease required.
Agency hours: 8:00AM-5:00PM, Monday-Friday.  Evening and weekend internship may be available (traveling to patient's homes, or in one of our Hospice Houses).
Placement in the BV Department is also available.
Employee orientation (8:00AM-5:00PM, Monday-Friday for two weeks) attendance required.
Interns are required to learn the organization's electronic medical record program.
