LifeSpan Case Management, LLC provides medical and psychiatric case management services to patients over the age 18. 
Our case management approach in all cases involve a clinical Social 
Worker case manager leading the team. Our team range from ARNP’s- psychiatric/ family practice, Nurse case managers and Social Workers in conjunction with medical doctors and other providers.
LifeSpan has been in practice for over 30 years serving Leon and the surrounding counties. 
LifeSpan practice areas specializes in providing psychiatric and medical services to patients in their homes, LTC placements and hospital settings. 
LifeSpan case managers offer a vast array of 
trained personal in specific areas of expertise. Court evaluators to determine competency, expert witness in psychiatric cases in the court system. 
LifeSpan’s LCSW’s offer family, individual, and group psychotherapy in our patient population. LifeSpan clinical approach is a team model to achieve the best outcomes for our patients in the least restrictive environment.

Special placement information:
Accepting BSW, MSW 1st-year, and MSW 2nd-year/Advanced Standing (SL + clinical) students. Full- and part-time positions available.
Transportation required.
Agency hours: 9:00AM-5:00PM, Monday-Friday. Weekend and evening hours available.
COVID-19 vaccine required.
