Our NC Community Alternatives Program is a Medicaid waiver program serving disabled adults (21-120 years of age) and medically fragile children (birth - 21st birth date). These services were designed to deter or end permanent institutional care. Our agency provides case management services for approved CAP beneficiaries. Each case has an assigned caseload to serve. CAP services are provided in the beneficiary's private residence where health, safety, and well being must be maintained.
