Information

St Augustine Youth Services (SAYS) is a mental health agency that provides residential and community-based services.  The residential program consists of three specialized therapeutic group homes and one traditional foster care group home.  The homes serve males ages 6-17 and provide a full array of therapeutic services including assessment, medication management, treatment planning, social and life skills training, crisis intervention, case management, individual, family and group therapy.

SAYS has three community-based programs:  COACHES, Community Action Team and Mobile Response Team.

The COACHES program is a case management program targeting youth and young adults needing assistance with self sufficiency goals.  The case manages provide care coordination utilizing High Fidelity wraparound to help with life domains such as education, employment housing transportation, and health.  The community Action Team provides in-home services to youth and families requiring intervention to stabilize the home.  Each client will be assigned a team that includes a therapist, case manager family mentor, and peer specialist.  A psychiatrist and nurse are also available to assist with medical and mental health needs.  Finally, the Mobile Response Team operates a hotline 24/7/365 in St Johns County for mental health crisis calls.  The team consists of licensed clinicians tat will respond on-site to calls and provide the necessary level of intervention.  A risk assessment is also conducted to determine the need for placement in a crisis stabilization unit.  In addition, the MRT team includes a case manage to help with other identified needs with the goal of preventing future crisis events.
