Elderly Population end state Renal Disease social worker provides emotional support to our elderly frail patients perform psychosocial assessment.  Obtain KDQOL annually, provides monthly psychosocial progress notes.  Process patients transplant document.  Be a part of the team formulating short term and long-term plan.

Special Placement Information:

MSW 1^st year students, MSW 2^nd year and Advanced Standing, (clinical) Part-time MSW.  Students will need liability insurance audio and video taping is not allowed.  Our hours are MWF 5:00am to 4:00pm.  Sat 5:00am to 1:00pm.  Pembroke Pines Location.
