The Tallahassee Pediatric Behavioral Health Center (Center) is a strategic initiative by Whole Child Leon in partnership with the Tallahassee Pediatric Foundation (TPF).
The Center was created to improve access to quality behavioral health services for children and their families through integrated health practices. The Center’s flagship program is the Behavioral Health Navigator Program.
Finding the right mental and behavioral health assistance for a child can be difficult, overwhelming and time-consuming. Our Pediatric Behavioral Health Navigators (BHNs) are trained clinicians who work directly with community providers and partners to ensure quality behavioral health services are
available and accessible when families need them. The BHN services are free of change and available to children, 0-21 years, who have a primary care medical home in Leon County. The BHNs use a child-centered approach to assist families in developing a tailored and comprehensive treatment plan, addressing the needs of the whole child and the whole family. To ensure success, BHNs work with families to address any barriers that limit access to quality care. In addition, BHNs regularly communicate with the child’s referring primary care physician so services and care are fully integrated between the Center and the doctor’s office.
Intern job description:
The Behavioral Health Navigator (BHN) Intern, under the supervision of the clinical supervisor and BHN Lead, works directly with the pediatric primary care community to provide resources and assistance to children and families in need of behavioral health and social services.
The BHN Intern assists referred families in navigating obstacles to behavioral health care access in order to connect them to high-quality preventative services, diagnosis, timely treatment, and follow-up. The BHN Intern utilizes and assists in keeping current a robust, vetted system of providers and community partners to help children and families connect with the most appropriate resources and participates in events and educational opportunities to advance the role of an integrated health model in improving health and wellness outcomes for the pediatric population.
Responsibilities:
• Conduct appropriate assessments of each child/family referred to the program to
determine needs
• Work with supervisor to create individualized navigation/case management plan for
each client including referral coordination to behavioral health providers and other
supports
• Assist families with accessing referrals (e.g., arranging transportation to and from
appointments, securing and maintaining insurance, etc.)
• Maintain regular contact with clients to ensure referral utilization, progress, and
identify additional needs; contact should be more frequent for those needing
additional support
• Assist with regular outreach efforts to establish and maintain positive working
relationships with physicians, social services staff, and the broader community
• Maintain timely and accurate electronic client records
• Develop and keep current a comprehensive listing of area behavioral health providers
• Serve as an essential link between clients and all other care providers
• Facilitate communication and follow-up with referring health care providers
• Reports to the Clinical Supervisor and Behavioral Health Navigator Program Lead
• Participate in site-based staff meetings and interdisciplinary project teams
• Comply with all pertinent regulations including HIPAA
