UF & Shands Jacksonville is an academic, not-for-profit, 733-bed medical center. We provide a wide range of healthcare services on an inpatient and outpatient basis. Backed up by a team of more than 330 faculty physicians, we offer nearly 70 specialty services. Specialties/Services include, but are not limited to: a Level I Trauma Center, a Level III Neonatal Intensive Care Unit, and a Transitional Care Unit. We currently maintain a contract with the City of Jacksonville to provide healthcare services for uninsured residents of Duval County. Role of Social Worker: Within the Department of Case Management at Shands Jacksonville, there are 9 Utilization Review Nurses and 33 Case Managers (with nursing and social work backgrounds). The department’s mission is to promote optimal patient outcomes throughout the continuum of care and assure that appropriate services are provided in an efficient and effective manner. In striving to fulfill our mission, we are guided by the C.A.R.E.S. model: Collaborating throughout the continuum of care, Advocating for our patients and families, Resource and utilization management, Educating and empowering our patient and families, hospital staff, and each other, Supporting performance improvement initiatives. The primary function of social workers (case managers) within the Department of Case Management is to facilitate patient throughput, patient advocacy and patient discharge planning. Social workers complete psychosocial assessments and document interventions in medical record, attend medical team rounds, coordinate discharge plan, provide community resources, act as patient advocates, and collaborate with interdisciplinary team.
Special Information:
Students will be responsible for own transportation, but will not be responsible for transporting patients, or families. We are accessible by city bus. At this time, a stipend is not available. To maintain compliance with Federal regulations (HIPAA) video and audiotaping of patients will not be permitted. Our hours of operation are 7 days a week 8:30 – 5:00. At this time evening hours are not available, but consideration may be given to student needs.
