Information
We provide hospice and palliative care to individuals with life limited illness, including children. We also provide comprehensive grief support to hospice survivors as well as those individuals that have experienced death of loved one, unexpectedly. The Social Worker for hospice care provides case management, end of life counseling and resources. They work within an interdisciplinary team including MD, RN and Chaplains. We also provide support and counseling to the family members of the patient.
Special Placement Information:
The Agency accepts MSW 1st year students, MSW 2nd year and Advanced Standing Clinical Students, Part-time MSW. Depending on the circumstances, we may be able to accommodate a paid internship
