The Department of Integrated Family Support Services provides ambulatory health-related social services for patients struggling with comorbid medical, social, and environmental complexity. Master’s level social workers embedded within primary and specialty care clinics, emergency rooms, and the community provide social services. Patient populations are generally low-income, range across multiple racial/ethnic groups, and cover the lifespan. Service areas include women's health, older adults, oncology, internal medicine, and family medicine. Working in an integrated healthcare environment, our team of social workers provides a wide range of social work interventions and activities: case management care coordination, health literacy, care conferencing, behavior coaching, supportive counseling, advocacy, and participate in quality improvement.
