Health Connect America, a Foster Care Program, offers community-based services. The agency works with foster parents in the community.
The agency licenses and maintains foster homes, ensuring that they meet all DCF and company standards. The agency places children ages 0-17 into these foster homes from various cultures and ethnicities. The social worker's responsibility is to license the home, provide support to the foster parent and the children in the home (providing and referring to any services that may be needed), complete assessments, develop, plans for the children, stabilize the placement, and anything else that may be needed.

Special Placement Information:

The Agency is accepting MSW 2nd year and advanced standing.  Students are expected to drive their own vehicle, maintain a good driving record, and maintain vehicle insurance and registration, as they will be using their vehicle to make home visits.  Agency office hours are 8 am to 5 pm and weekend/evening hours are available at the student will make their own schedule when conducting home visits.  There is also an IOP group that meets at the office on Monday, Tuesday, and Thursdays from 4:30-7: 30 pm.  The agency is accessible by transportation buses.
