HSNCF subcontracts with direct service providers to provide services to pregnant women and families with children up to age 3. Care Coordinators conduct home visits with the families and provide education, support, and information. HSNCF received a grant to pilot a Coordinated Intake and Referral Program for Alachua County. Through this program, a “Family Partner” meets with families at the hospital (UF Shands) and UF Children’s Medical Services to determine if the family needs assistance and links them to the supports and services that will help them succeed as parents/caregivers.

BSW and MSW (1st year)
Student will need transportation to either Children’s Medical Services or UF Shands. Travel will not be reimbursed and there is not stipend available at this time. No video or audio recording is permitted without the written consent of the participants. The hours for the student are flexible and weekend hours are available at UF Shands.
