Kindred Hospice Palm Coast services a diverse adult population throughout the end-of-life process. Majority of the population serviced is the geriatric population. The role of the Social Worker within the hospice setting is to assess the family, caregivers and patients for any pre-bereavement or psychosocial needs. Social Workers assist patients and families with community resources to assist with end-of-life care or placement. Social Workers may assist with Medicaid applications for Long Term placement. Social Workers provide information concerning funeral homes and Advanced Directives while collaborating with the RN case manager. The Social Worker attends Interdisciplinary Group meetings to present Code Status, disaster planning and any psychosocial needs that may be present with the family or patient.

Special Placement Information:

The Agency is accepting BSW Full-time, MSW 1st year student, MSW 2nd year and Advanced Standing, (Clinical) Part-time BSW, Part-time MSW.  The agency hours are Monday through Friday - 8:00am - 5:00pm. The student will meet the Social Worker at the office and follow them to each patient's home in their own personal vehicle. The Social Worker then will meet with the student after each visit to discuss each patient situation and resources available to meet the needs of the patient. Video or audio taping is not permitted during the visit by the student.
