Agency Description:

The primary function of a medical hospice social worker in hospice care is to provide resources to patients and their families during the final stages of their life. Many do not know about everything provided in hospice care and the support available such as available group homes and assisted living o meet their needs. Others need help with the Medicaid applications and completion of advance directives and DN living wills and powers of attorney, funeral services and bereavement support and service. 

Special Information:

Accepting full-time and Part-time MSW 1st year and 2nd Year/Advanced Standing students. TB Test required, fingerprint clearance, CPR certification. Mileage will be reimbursed at 55 cents per mile. As social workers in hospice care, we travel often to patients' homes. Navigation/cell phone/ and patients sign photo release consents so pictures are allowed. Hospice social workers are generally answering questions via email, text, or call and it can be on weekends as well. Visits are made however Monday-Friday.
