Patients must have a diagnosis of six months or less to live. We have a team of medical professionals who will focus on the physical, emotional, psychological, and spiritual needs of our patients.
The team consists of a Medical Director, nurses, CNAs, Social Workers, Chaplains, a Volunteer Coordinator, and therapists.
Our mission is pain management and symptom management. We support the patient and family and allow them to manage their life the way they want in their last days.
Intern job description:
Intern will assist the professional Licensed Social Worker in all task-related to their practice. This includes psychosocial assessments, development of plans of care, identifying strengths and coping skills, education to patients and families about end of life, assessing safety concerns, maintaining records, participation in interdisciplinary group meetings, collaborating with community resources, and participating in training and development.
Special placement information:
Accepting MSW 1st-year and MSW 2nd-year/Advanced Standing (SL + clinical) students. Full-time positions only.
