Information

The main function of TMRC is to provide services to patients in need of inpatient sub¬acute rehabilitation. The primary populations served are individuals who reside in Florida, southern Georgia and southern Alabama. The age of patients who seek services at TMRC range from late teenage years to elderly individuals. The main purpose of TMRC is to rehabilitate patients so they may hopefully return to their previous level of functioning. TMRC primarily targets to three levels of rehabilitative patients: Orthopedic, Neurological and Medical. Orthopedic patients include individuals who have broken bones, joint replacements or other surgeries to improve physical functionality. Neurological patients include individuals who have had strokes, or other head/neurological injuries resulting in impairment of cognitive functioning. Medical patients include individuals who have had major medical problems, such as heart attacks or other severe internal complications, or major wounds/infections in need of constant care. TMRC utilizes social work, physicians (specialists and generalists), physical/occupational/speech therapy, nursing services, activities, music therapy, and support/educational services to aid in rehabilitation. The role of the social worker is to provide services to both patients and families. The social worker advocates for patients and their families to ensure services, treatment and resources become available if needed during treatment and post discharge. The social worker completes assessments and treatment plans on every patient who is admitted to TMRC. The social worker also works clinically with patients and families regarding anxiety, depression, coping and adjustment, if needed. The social worker has an opportunity to do individual and family psychotherapy. The social worker also works within an interdisciplinary format to help patients accomplish treatment goals and return individuals to their previous level of functioning. TMH-Rehab Center: Focus is on rehabilitation but we fall under SNF guidelines and paperwork (not acute hospital paperwork)
1) Psychosocial assessments to be done for each patient (up to 20/week)
2) Discharge planning and community reference for all discharged patients (up to 20/week)
3) Counseling as needed
4) State paperwork guidelines (MDS) completed for each patient
5) Medicaid, transportation, other resource linkage

* Population: varies from 20-90's, mostly middle-age
